The Living Well Community Health Development (LW CHD) programme is based on the ‘Whole systems Approach to community centred public health’ framework.
It utilises a community-centred approach to reduce health inequalities and strengthen community participation for health and wellbeing. The team includes community development and engagement workers situated across localities, to engage local people in activities to reduce health inequalities.
The programme aims to increase equitable delivery of Living Well (our whole systems approach to obesity, health and wellbeing) and engage at a grassroots level with local communities.
The programme and its staff work closely with wider system partners and are integrated into the local authority’s Public Health and Neighbourhoods teams, with three broad remits to:
a) Engage communities and maximise opportunities available through the Living Well Programme to reduce health inequalities;
b) Facilitate and sustain asset-based community development approaches in areas of greatest need;
c) Gather insight into barriers and facilitators for health and wellbeing directly from communities across the District to feed into strategic planning of local health and social care.
Another important aspect of the programme is to offer Community Health Development grants to voluntary sector organisations. The purpose of these grants is to give a voice to and improve health within communities. The grants aim to engage and build capacity within communities to support activities that address inequalities in health and wellbeing of underserved populations.
Our friendly community development & engagement workers, work within five Localities (geographical areas). You may see us out in the parks, at community centres, health centres, libraries or at the many events which take place in our communities. If you see us, come and say hello! We also offer information about Living Well and introduce you to the many Living Well offers.
In the past few months we have helped to develop walking groups, repurpose green space for allotment use, men’s groups and tools workshops, support parent & toddler groups & healthy cafés, women’s wellbeing taster workshops, play streets and coproducing activities with our Start4Life and Health & Wellbeing Hub partners.
