What is the stop smoking incentive offer?

The Living Well Stop Smoking Service is launching an exciting new pilot offer to help people in Bradford to stop smoking.

Join our 12-week treatment programme and earn up to £100 in Love2Shop high street gift vouchers!

To be eligible for this offer you must meet the criteria below and be registered as a patient at one of the following GP practices:

Tong Medical Practice

Rooley Lane Medical Practice

Low Moor Surgery

Bowling Hall Medical Practice

Eligibility criteria:

If you currently smoke and would like to quit with this offer, you must:

  • Be a registered patient at one of the 4 medical practices within BD4+ mentioned above.
  • Be at least 18 years old.
  • Be committed to stopping smoking and participate in a 12-week treatment programme offered by our Living Well Stop Smoking Service.
  • Attend a face-to-face initial appointment offered within the local BD4 area and further appointments either face to face or via telephone as agreed with advisor.
  • Provide a Carbon Monoxide reading to confirm outcomes at your initial consultation, at your week four consultation and again at your final week twelve consultation.
  • The voucher incentive scheme is for £100 in total. 1 quit attempt per person who meets the criteria listed above.

How will I receive my vouchers?

Voucher 1 – £20 Love2Shop voucher
You will receive this voucher when you set a quit date to stop smoking

Voucher 2 – £30 Love2Shop voucher
You will receive this voucher when your week 4 carbon monoxide check is validated as a successful quit

Voucher 3 – £50 Love2Shop voucher
You will receive this voucher when your week 12 carbon monoxide check is validated as a successful quit
