Stop Smoking Incentive Offer Terms and Conditions

1. The Stop Smoking Incentive Scheme will be delivered between 1 January 2026 and 31 March 2026. Participants must have their 1^st appointment booked within these dates.
2. The Stop Smoking Incentive Scheme is only available to patients invited by their Medical Practice or by Stop Smoking Services. The invite will be in the form of an SMS message, and the Stop Smoking Services may ask to be shown the SMS to prove identity.
3. All E-Gift Cards have a 6-month expiry date and must be redeemed with this timeframe.
4. This is a one-time incentive and if any of the following occurs, the incentive scheme will be withdrawn but any previous E-Gift Cards allocated can be redeemed until the expiry date.
   a) Missed appointment
   b) Positive Carbon Monoxide verification
   c) Patient withdraws from the incentive scheme
5. Patients can only participate in the Stop Smoking Incentive scheme once. There are no multiple quit attempts
6. Love2Shop E-Gift Cards are not transferable between individuals.
7. E-Gift Card purchased are monitored and cannot be redeemed against inappropriate products such as:
   a) Alcohol products
   b) Nicotine products
   c) Fast food delivery services
   d) Redeem against foreign currencies
   e) Gambling products including in-play gaming credits.
8. The Love2Shop E-Gift Cards will be cancelled if any of the following occurs:
   a) Bradford Council are made aware of the purchase of inappropriate products.
   b) E-Gift Cards have been used by anyone other than the person participating in the incentive scheme.
   c) E-Gift Cards are transferred to anyone other than the person participating in the incentive scheme.
